- Build their capacity and ability to design, develop and implement their supports;
- Collaborate with clients to establish person-centered goals and aspirations;
- Understand their plan and what supports are available in their plan;
- Assist with accessing the NDIS portal;
- Link with the most appropriate services to assist them to meet their NDIS goals;
- Coordinate services in their community to best meet their needs and goals with the development of service agreements, service providers and obtain quotes for supports;
- Resolve issues or conflicts which may arise with providers and coach clients through challenges;
- By monitoring their plan outcomes and provide appropriate reports, and prepare for their plan review.
